• Please be patient. Sometimes appointments run a little longer. I may be 5-15 minutes late. Know that I will ensure quality time is spent addressing your concerns.
• Paid underground parking is available in the back lane of Staples
• There is no receptionist. Please check in on the iPad in the waiting room and have a seat. I’ll be out shortly to get you.
• If you can no longer make this appointment, please call, text, email, or visit your account page to cancel.
• Please DO NOT take the stairs to get to the 4th floor. They are emergency exits and are only meant for exiting the building.

• Paid and Free 2-hour street parking is available on Hemlock Street, Birch Street, and 10th Ave.

Extended Health Benefits Insurance:

• Please check with your provider to ensure you have coverage before your appointment. Many plans require a doctor’s note for coverage.
