• When you arrive, please have a seat in the waiting area, and I’ll be out to come and get you.

• Please be patient, sometimes appointments run a little longer, and I may be 5-15 minutes late. Know that I will ensure quality time is spent with you.

Parking:

• There is paid underground parking available, which can be accessed directly behind the building, either by entering from the South side of Hemlock Street or the South side of Birch Street.

• Paid and Free 2-hour street parking is available on Hemlock Street, Birch Street, and 10th Ave.

Extended Health Benefits Insurance:

• Please check with your provider to ensure you have coverage before your appointment. Many plans require a doctor’s note for coverage.
